Penile curvature is a common condition in men and is defined as a deviation from the penis's natural position or during erection. Peyronie's Disease

Peyronie's disease is a condition characterized by the formation of fibrous plaques in the penis. These plaques create a curvature on one side of the penis during erection, making it difficult to achieve an erection. Although Peyronie's disease is most common in men over 40, it can also occur in younger men. The condition can be painful and cause a noticeable curvature during erection. While the exact cause of Peyronie's disease is unknown, it is thought to be related to traumatic injuries or autoimmune diseases.

For patients with early-stage Peyronie's disease, medical treatments, particularly oral medications and nonsurgical options such as penile injections, may be available. However, in more advanced cases, surgery may be necessary.

Congenital Penile Curvature (Chorus)

Congenital penile curvature, or chordee, is a structural abnormality of the penis present from birth. This condition results from abnormalities that occur during the normal development of the penis. Improper development of the penis's elastic tissue, scar tissue formation, or improper placement of connective tissue can cause congenital penile curvature. This type of curvature is usually noticed after sexual maturity and can lead to serious sexual problems.

Congenital penile curvature is usually corrected surgically. Surgeons manipulate scar tissue and ligaments in the penis to correct the curvature, allowing the penis to function properly.

Penile Fracture

A penile fracture is a rare but serious injury that occurs when the penis is subjected to sudden and extreme bending while erect. The tissue in the inner structure of the penis, called the tunica albuginea, can tear as a result of the bending of the penis. Penile fractures usually occur during sexual intercourse, but can also occur from sports accidents, traffic accidents, or other physical trauma. A penile fracture is a condition that requires immediate surgery and can lead to permanent curvature and erectile dysfunction if left untreated.

Autoimmune Diseases and Connective Tissue Problems

Connective tissue is a key element that forms the penis's supporting structure. Disorders in connective tissue can cause penile curvature. Autoimmune diseases occur when the immune system attacks healthy body tissues. Autoimmune conditions such as Peyronie's disease, lupus, and Behçet's syndrome can contribute to the development of penile curvature. Examples of connective tissue disorders include Dupuytren's contracture and plantar fasciitis. These conditions typically affect both the penis and other body parts.

Risk Factors for Penile Curvature

  • Surgery or radiation therapy after prostate cancer treatment

  • Excessive or violent sexual activities

  • Family history of Peyronie's disease

  • Presence of diabetes and erectile dysfunction

Penile Curvature Treatment Options

Treatment for penile curvature varies depending on the cause and severity of the curvature. Treatment options include nonsurgical and surgical interventions. If the curvature is mild and not causing pain, medical observation and symptom management may be sufficient.

Pain Management:

Anti-inflammatory medications such as ibuprofen may be used to relieve pain.
Penile injections: Medications used for Peyronie's disease can help shrink scar tissue in the penis and reduce curvature.

Surgical intervention:

In cases where the curvature is severe, especially in Peyronie's disease, scar tissue in the penis can be removed and the curvature corrected through surgical intervention.

Who is Suitable for Surgical Intervention?

Patients who experience severe pain or loss of sexual performance and who have not benefited from nonsurgical methods are good candidates for surgical intervention. Surgical intervention can improve the patient's quality of life both physically and psychologically by restoring the natural form of the penis.

Who is Not Suitable for Surgical Intervention?

Surgery is generally not recommended for individuals with mild or non-painful curvatures. Penile traction devices or nonsurgical treatments may be considered instead. In mild cases, these treatment options can be quite effective.

Surgical Intervention Process

During surgery, scar tissue is removed from the penis, restoring its natural shape. This surgical procedure, used for Peyronie's disease, is considered the gold standard for curvature correction. While recovery time varies from person to person, patients can generally return to their normal sexual lives within a few weeks.
