How Is Penile Prosthesis Surgery Performed? Step by Step

How is penile prosthesis surgery performed? Anaesthesia, incision, placement of the implant, duration and recovery. Assoc. Prof. Zülfü Sertkaya, Istanbul.

Penile prosthesis surgery is a procedure for lasting erectile dysfunction that does not respond to medication or regenerative treatments, in which a device providing an on-demand erection is placed inside the penis. It is known for high patient satisfaction and, with the right indication, offers a lasting solution.

What is done before surgery?

A detailed andrology assessment is performed; the cause of ED, coexisting conditions (diabetes, vascular disease) and expectations are evaluated. The suitable implant type (malleable or inflatable) is chosen together with the patient, and preparations to reduce infection risk are completed.

How does the operation proceed, step by step?

  1. Anaesthesia: usually general or spinal anaesthesia.
  2. Incision: a small incision is made via an infrapubic or penoscrotal approach depending on the implant.
  3. Placement: the erectile bodies (corpora cavernosa) are prepared and the cylinders placed within them.
  4. Inflatable models: a reservoir and pump are positioned and the system is tested.
  5. Closure: the incision is closed so the device remains invisible from outside.

Why is the no-touch technique important?

A key goal is minimising infection risk. The no-touch technique places the implant without contact with skin or the environment, aiming to lower infection risk. Combined with antibiotic-coated implants and meticulous sterilisation, low infection rates are targeted.

How long does it take; is a hospital stay needed?

Duration varies by implant and case, usually around one to two hours. A short hospital stay is typically sufficient. Device use is explained in detail after healing.

How does recovery progress?

Mild swelling and tenderness are normal in the first days. Following the recommended care and follow-up is the most important part of recovery; most patients return to daily life within a short time.

Assoc. Prof. Zülfü Sertkaya is one of Europe's leading high-volume penile implant surgeons, holding Boston Scientific AMS 700™ and Coloplast Titan® Center of Excellence credentials and operating with the no-touch technique, in which the implant is placed without skin contact. At his clinic in Istanbul, all assessments are carried out confidentially.

Frequently Asked Questions

The procedure is done under anaesthesia, so no pain is felt during surgery. Mild post-operative discomfort is easily managed with prescribed painkillers and eases within a few days.
No. Because it is placed entirely inside the penis, it is not visible; inflatable models preserve a natural appearance when deflated.
This depends on healing and implant type; a recovery period of a few weeks is usually advised, confirmed by your surgeon at follow-up.
The prosthesis does not affect urination; ejaculatory sensation is preserved in most patients.
